New VG Downloads:
The following form needs to be completed by a new VG in order to get them started in the program. There are two formats from which to choose:
PDF print: download, print, complete by hand and physically send by fax or ground mail;
MS Word form: download, complete on your computer, save, and submit via email, fax or ground mail.

All VGs need to complete this for publishing on the VGP Web site. It includes a short bio of about 300 words and topics by which VGs may be sorted within the VGP site. Also it has room for you to tell us about your travel limitations and availability to aid in scheduling visits.
