New VG Downloads:
The following forms need to be completed by a new VG in order to get them started in the program. Each document is available in one of the following formats:
- PDF form: download, complete on your computer and submit electornically;
- PDF print: download, print, complete by hand and physically send by fax or ground mail;
- MS Word form: download, complete on your computer, save, and submit via email, fax or ground mail.
Documents for completion are:
- Speaker Nomination Form: PDF | PDF Print | MS Word
- Provides the background, topics, and other pertinent information for a potential VG.
- Topic/Bio Form PDF | PDF Print | MS Word
- All VGs need to complete this for publishing on the VGP Web site. It includes a short bio of about 50 words and topics by which VGs may be sorted within the VGP site.
- Speaker Questionnaire PDF | PDF Print | MS Word
- Tells us about your travel limitations and availability to aid in scheduling visits.
