Form Instructions

The forms are provided here in three formats: PDF print only, PDF Form and Microsoft Word Form. You will need Adobe Acrobat Reader to view the PDF (portable document format). If you have Windows or Mac OS you will need Microsoft Word 6.x or up to work with these files.

In either case the best way to navigate through a form will be to use your tab key. However, for the mouse-dependent, that's okay, too.

Adobe Acrobat Reader is provided as a free download on the Adobe Web site.

 Online forms

The online forms allow you to complete a form in your favorite browser. Upon submittal the information you completed then goes to AAPG headquarters. Should you be interrupted while using thie form and lose the information you will have to start again so it is advisable to prepare your information in a word processing document so you may simply copy and paste your information into your form.

 PDF print only

The PDF files may be printed only. You may then fill them out by hand either printing or use a typewriter.

 PDF Form

The PDF files may be viewed and completed on your computer using the fields provided. A submit button sends the data to AAPG for processing. It is important that you realize these files cannot be saved if you are viewing them through Acrobat Reader. So, to maintain a record of what you are submitting, please print your document before closing. Once you quit the application all that you have written will be lost if you did not print a hard copy of the file.

What's the advantage? In this new computer age many people do not have access to a typewriter and if one's handwriting is difficult to read the legibility of the completed form becomes a challenge.

 Microsoft Word Form

A MS Word Form has been created in Microsoft Word. The document can be opened and form fields are present to accommodate the completion of the form. No other text may be edited on the form. This form, however, may be saved on your computer. The ability to save the form opens up the option of sending a file via e-mail attachment to AAPG. You may still print the form and send via fax or mail; however, the e-mail option is recommended.

In either case, PDF or MSWord, a completed Commitment Statement -- includes name of candidate and you, the submitter's name -- will need to be printed and signed before returning to AAPG. Electronic versions of this form cannot be accepted.

 The Forms

Each nomination form is different for each office so be sure you are using the correct form for your nomination. You may nominate a person to only one office. (See Executive Committee Instructions) Simply click on the link provided below to use the form of your choice.
