From the Honors and Awards Home page you can read the description and criteria about each award. Because of this uniqueness you must use the appropriate form to nominate someone for that award. Thus, if you wish to nominate an individual to be considered for three different awards you must fill out a form for each award.

TIP: It is recommended that you print a PDF of the award's form before using the online submittal form.
The HTML online form allows you to fill out a form through your browser and then you may print out the information and fax or mail to AAPG. There is no ability to save this data so once you leave the page, the information will be gone. On the HTML pages are links to the other three formats for that award.
The Microsoft Word form is compatible with Mac or PC. It has form fields for the data being requested for each nominee. When you open this form you should first save the document with a new name (use the Save As ...command in the File menu). Throughout your completion of this form you may save your document until you are ready to submit it to AAPG. This form may be emailed as an attachment or printed and then mailed or faxed.
