Please read each nomination form carefully. It is important you select the correct form for your nomination and mark it appropriately for the office of choice. A person may be nominated to only one office. (See Executive Committee Instructions) Simply click on the link provided below to use the form of your choice.
The forms are provided here in three formats: PDF print only, PDF Form and Microsoft Word Form. You will need PDF viewer to access the PDF (portable document format). We recommend Adobe Acrobat Reader. If you have Windows or Mac OS you will need Microsoft Word 6.x or up to work with these files.
The completed AAPG Officer Nomination Form and the Commitment Statement for AAPG Office should be forwarded to Karin Alyea and Vicki Beighle, AAPG, P.O. Box 979, Tulsa, OK 74101 by 1 August 2017. Nominations received after 1 August will be held for next year’s officer nominations.

- PDF print only
- The PDF files may be printed only. You may then fill them out by printing clearly or use a typewriter.
- PDF Form
- These PDF files may be completed on your computer. It is important that you realize these files cannot be saved if you are using a PDF reader. Once you quit the application all you have written will be lost if you did not print a hard copy of the file before quitting.
- Microsoft Word Form
- A form has been created in Microsoft Word. The document has form fields activated for completion on your computer. No other text may be edited on the form. This form may be saved on your computer. This opens up the option of sending a file via e-mail attachment to AAPG. You may still print the form and send via fax or mail.
TIP: The best way to navigate through interactive forms will be to use your tab key or mouse.
